Notes for the doctor about my teen's worry
- What my teen worries about most, in their words if possible
- When it started, and anything that happened around then (move, loss, detention, bullying)
- Body signs: stomachaches, headaches, sleep, heart racing
- What they avoid: school, classes, friends, places, people
- How many school days missed in the last month
- Any rituals they feel they must do (washing, checking, counting)
- Alcohol, vapes, drugs or medicines, including from another country
- What I do to help that might keep the fear going (answering for them, letting them stay home)
- Questions: "Is this anxiety, OCD or something else?" "What therapy do you suggest?" "How can I help at home?" "What is kept private?"
- The language our family wants to use
If your teen talks about dying, call or text 988. If a life is in danger right now, call 911. You do not have to write your immigration status on this sheet.
