Update order checklist
- Court order (Decree Changing Name, NC-230 or NC-330). Get several certified copies.
- Social Security name change (1-800-772-1213). Keep the receipt.
- DMV: apply online, then visit a field office. Bring current card, court order, REAL ID documents.
- Birth certificate (CDPH) and marriage record, if needed.
- Immigration documents: I-90 for a green card. Ask a lawyer about anything else.
- Passport (U.S. citizens): DS-5504, DS-82 or DS-11.
- Bank, employer, school, health plan, Medi-Cal, IRS or FTB, insurance, phone, utilities.
- Keep one folder with every certified copy and receipt.
