Before you start

• My DACA ends on: ____________. Today is 120 to 150 days before that date (earlier is accepted, but may cost me some DACA time).
• My DACA has not been ended by USCIS, and it ended less than one year ago if it already ended.
• I talked with a clinic first if I was arrested, charged or put in immigration court since my last renewal.

Forms (printed single sided, same edition on every page)

• Form I-821D, edition 01/20/25, signed by hand in ink
• Form I-765, edition 08/21/25, category (c)(33), signed by hand in ink
• Form I-765WS worksheet
• Name, birth date and address written the same way on every form
• Part 4 of the I-821D: questions 1 to 7 each marked Yes or No

Payment (pick one type for the whole packet)

• Card: Form G-1450, or bank: Form G-1650. Not both.
• Two payment forms of the same type: $85 for the I-821D and $520 for the I-765 ($605 total). Exact amounts written in.
• Payment forms signed. Enough money in the account. Bank told about USCIS code 7001010330 if my account blocks withdrawals.

• 4. Form G-28 (only if an attorney or accredited representative helps)
