Print this page and check off each step. Do not write immigration details, case numbers or anything private on it.
- I chose a trusted adult and talked with them.
- I chose a second adult, just in case.
- We filled out the Caregiver's Authorization Affidavit on the January 2026 form, one for each child.
- My caregiver, the school and I each have a copy.
- I updated the emergency contacts at school and child care.
- I gathered birth certificates, passports and health information in one safe place.
- My caregiver knows where those papers are.
- My children and I know at least one important phone number by heart.
- I decided who could handle rent, bills or the car, and asked a lawyer about a power of attorney if needed.
- I asked the court self help center about guardianship, if I want someone to have legal custody.
- We will look at this plan again in six months.
