How to ask for leave (say it or send it in writing)
"Hello __. I need to take time off from work starting //.
The reason is (choose one):
- my own health condition
- my pregnancy
- to bond with my new child
- to care for my family member, who has a serious health condition
- a death in my family
- an injury I got at work
I expect to return on about //______. I will give you a note from my health care provider if you need one.
I am asking for this leave under California law. Please tell me in writing whether my leave is approved, whether my job will be held, and whether my health insurance continues.
If I was hurt at work, please give me the workers' compensation claim form (DWC 1).
Thank you."
