- I am not on a valid student, work or other nonimmigrant visa
- I got SSS Form 1 at the post office (or printed it from sss.gov)
- I used my legal name, date of birth and a safe mailing address
- I left the Social Security number blank because I do not have one
- I asked for proof of mailing and kept a copy
