Forms
- Form W-7, newest version from irs.gov
- My federal tax return (Form 1040), with the SSN boxes left blank for each ITIN applicant
Papers (originals or certified copies, current today)
- Passport, or
- Two papers, one with my photo (for example: national ID card plus birth certificate)
- For each child: passport, or birth certificate plus a second paper, and proof they live in the U.S. (ask a tax helper)
Where I will apply

After
- Copy of everything I sent
- Watch for the IRS letter with my ITIN, in about 7 weeks (9 to 11 weeks in tax season)
