- Proof of my case (receipt notice, approval or parole papers)
- Copy of my current or last work permit, front and back (every renewal)
- Two identical color passport style photos
- Fees checked on uscis.gov today, including any separate federal fee
- Paper filing: current address from the I-765 address page, and G-1450 or G-1650 for payment
- Form signed
