Keep copies in a safe place. Give them to your lawyer, not to anyone else, unless your lawyer says to.
Program records
- Name, address and phone of each program or clinic
- Start and end dates
- Letters showing I took part and finished
- Counselor or doctor letters about my progress
Proof of recovery
- Drug or alcohol test results, with dates
- Support group attendance records
- Prescriptions for any medication for addiction treatment
