N-648 checklist for you and your doctor
Before the visit
- I checked whether the age exceptions (50/20, 55/15, 65/20) already cover me.
- My doctor is an MD, DO or clinical psychologist, licensed in the U.S.
- I am bringing records of my condition and a list of medicines.
- I am bringing an interpreter if I need one, who will sign the form.
For the doctor (Form N-648, edition 09/25/24)
- Diagnosis of each condition, in plain words
- Lasted or expected to last 12 months or more
- Not caused by illegal drug use
- How it was diagnosed (clinical or lab methods)
- Date of my last exam
- How each condition stops me from learning or showing English, civics or both
- Which parts I cannot do: English, civics, or both. Oath question answered.
- Signed by doctor, by me or my guardian, and by the interpreter
Filing
- Doctor signed within 180 days before I file my N-400
- N-648 sent with my N-400 (or uploaded online)
- Accommodation requested at uscis.gov/accommodations, if needed
- Full copy saved
