First week
- Called the social worker. Name: ____________ Phone: ____________ Date: ________
- Said clearly: "I am the child's relative. I want to be considered for placement."
- Gave the names of every adult who lives in or is often in my home
- Filled out Form JV-285, Relative Information, and asked how to file it
- Asked for the next court date: ____________
- Talked with an immigration attorney before fingerprints, if anyone in my home has a deportation order, past removal or arrest record
Home visit
- ID ready (a foreign passport or consulate ID counts for the emergency records check)
- Bed, crib or car seat the child needs, or asked the county for help getting them
- Medicines, cleaning products and anything unsafe stored away
Approval
- Applied for Resource Family Approval
- Training scheduled (12 hours before approval)
- CPR and first aid planned (within 90 days after approval)
- Asked in writing about emergency caregiver funding and monthly payments
Ongoing
- Hearing dates on my calendar
- JV-290 ready to send the judge updates
- Foster Care Ombudsman number saved: 1-877-846-1602
